For most people with a herniated disk, low back pain is the initial symptom. This pain may last for a few days, then improve. It is often followed by the eventual onset of leg pain, numbness, or weakness. This leg pain typically extends below the knee, and often into the foot and ankle. It is described as moving from the back or buttock down the leg into the foot.

Symptoms

Symptoms may be one or all of the following:
  • Back pain
  • Leg and/or foot pain (sciatica)

Discectomy involves removing fragments of the disc that press on and irritate the spinal nerve root. Discectomy is the most common surgery performed for a low back disc herniation. Discectomies may be full operations (called “open discectomies”) or they may be minimally invasive procedures. Due to technological advances in medical/surgical techniques, devices and equipment, the trend in surgery is toward the minimally invasive. Two types of minimally invasive procedures are the microdiscectomy and percutaneous arthroscopic discectomy.

If you are deciding which kind of discectomy to have, consider the skill set of your potential surgeon. Some doctors may insist on performing an open discectomy because they haven’t been thoroughly training in the minimally invasive types. Others may specialize in one type of procedure over the other. Check your doctor’s competencies and compare them with those of other surgeons before deciding who will perform the procedure.

Most patients notice improvement over the first several weeks following surgery, but may also experience continued improvement over several months. Pain is typically the first symptom to improve, followed by improvement in overall strength of the leg, and then sensation. Most patients will slowly resume normal daily activities over the first several weeks following surgery.

Over the last several years, there has been extensive research on lumbar disk surgery and patient improvement. One of the most publicized research projects in this area is the Spinal Patient Outcomes Research Trial (SPORT). The study followed patients with herniated disk from across the country. Half were treated with conservative measures, and half with surgery.


The initial outcomes for patients treated with surgery were much better than those who followed conservative treatment, including improvement in pain relief and function. At the 2-year follow-up, patients treated with surgery again showed improvements over those treated conservatively. However, over the course of the study, numerous patients did change their treatments. 1. A laminectomy – removing the back part of the bone (called the lamina) over the spinal column. A laminectomy is performed to relieve nerve root compression (pinched nerve) on one or more nerve roots in the spinal column. The compressed nerve root often causes back and leg pain. A segment of the entire lamina can be removed to relieve pressure on a nerve..
2. A discectomy – removing a portion of a disc to relieve pressure on a nerve.
3. A spinal fusion – this involves the permanent fusion of two or more vertebrae for more stability, to correct a deformity or to relieve pain. The surgeon will harvest small pieces of bone from your hip or pelvic bone and place them between the vertebrae. In many cases they will use wires, rods, screws, metal cages or plates to provide immediate stability.
How is a laminectomy performed?

Step 1: A laminectomy is performed with the patient lying on his stomach or side and under general anaesthesia. The surgeon (an orthopaedic or neurosurgeon) reaches the spinal column through a small incision in the back.
Step 2: He will use a retractor to spread the muscles of the back apart in order to expose the bony lamina.
Step 3: He cuts away part of the lamina to uncover the ligamentum flavum – a ligament supporting the spinal column.
Step 4: In the next step he will cut an opening in the ligamentum flavum to reach the delicate spinal canal containing the compressed nerve.
Step 5: The compressed nerve can now be seen as well as the bundle of nerve fibres (known as the cauda equina) to which it is attached. It is now possible to identify the cause of compression: a bulging, ruptured or herniated disc, or perhaps a bone spur.
Step 6: The source of the pressure can now be removed. This may involve removing the bulging portion of the disc or the bony spurs and scar tissue. The herniated disc is removed after the compressed nerve has been gently retracted to one side. The surgeon will remove as much of the disc as is necessary to take pressure off the nerve. Sometimes a fragment of disc has moved and presses on the nerve root as it leaves the spinal canal. This will often cause more severe symptoms.
Step 7: With the cause of compression removed, the nerve can now begin to heal. The space created by removal of the disc will gradually fill with connective tissue. The incision is closed in several layers, from the inside outwards.
Step 8: The skin layer will be closed with steri-strips, sutures or skin clips. A dressing will be placed over the incision to protect the wound.
This operation is normally performed within one or two hours, depending on the number of levels that are decompressed.
After surgery
A plastic drain will run from inside the wound to remove any accumulating blood. In most cases, the drain can be removed on the second day after surgery.
An intravenous line to administer medication may remain connected through a vein in your hand or arm for two to three days.
Pain can and should be well controlled. Usually the acute pain subsides after a day or two. Pain may be most severe in the lower back. Leg pain may be caused by swelling of the previously compressed nerve and the trauma of the surgery. Muscle spasms across the back and down the legs are not uncommon and this can be relieved by muscle relaxants.
A physiotherapist will help you to begin standing and walking again, and show you how to get in and out of bed and how to sit, stand, and sleep.
 What about new and less invasive procedures?
The era of less invasive surgery has dawned – also in back surgery. Smaller keyhole incisions are replacing large surgical cuts. These less invasive techniques include:

  • Endoscopic discectomy – this technique employs a disposable scope that the surgeon inserts through a small incision. The protruded disc that is compressing the nerve is then removed using specially designed instruments.
  • Vertebroplasty – the injection of bone cement into a fractured vertebrae. The hardened cement will seal and stabilise the fracture and relieve pain. Herniated Disk - Spinal Disk Problems Treatment in India

Only a person who has experienced a damaged spinal disk understands the agony and helplessness it brings. The pain can be excruciating. Every movement seems to make it worse.

Spinal disks are rubbery pads between the vertebrae, the specialized bones that make up the spinal column. Doctors call them intervertebral disks. Each disk is a flat, circular capsule about an inch in diameter and one-quarter inch thick. They have a tough, fibrous, outer membrane (the annulus fibrosus), and an elastic core (the nucleus pulposus).
The disks are firmly embedded between the vertebrae and are held in place by the ligaments connecting the spinal bones and the surrounding sheaths of muscle. There is really little, if any, room for disks to slip or move. The points on which the vertebrae turn and move are called facet joints, which stick out like arched wings on either side of the rear part of the vertebrae. These facet joints are separate from the discs and keep the vertebrae from bending or twisting excessively, which could damage the spinal cord and the vital network of nerves that runs through the center of the spinal canal formed by the stack of vertebra.
The disk is sometimes described as a shock absorber for the spine, which makes it sound more flexible or pliable than it really is. While the disks do separate the vertebrae and keep them from rubbing together, they are far from spring-like. In children, they are gel- or fluid-filled sacs, but they begin to solidify as part of the normal aging process. By early adulthood, the blood supply to the disk has stopped, the soft inner material has begun to harden, and the disk is less elastic. Understanding Spinal Disk Problems --Herniated Disk

Under stress, a disk's inner material may swell, pushing through its tough outer membrane. The entire disk can  become distorted or bulge in spots. With an injury, all or part of the core material may protrude through the outer casing at a weak spot, pressing against surrounding nerves. If further activity or injury causes the membrane to rupture or tear, the disk material may further extrude, causing pressure on the spinal cord or the nerves that radiate from it. This may result in extreme pain. In the beginning, there may be spasms in the back or neck which will greatly limit your movement. If nerves are affected, you may develop pain that moves into a leg or an arm.
Disk herniation is most often the result of a gradual, aging-related wear and tear called disk degeneration. As you age, your spinal disks lose some of their water content. That makes them less flexible and more prone to tearing or rupturing with even a minor strain or twist.
Most people can't pinpoint the exact cause of their herniated disk. Sometimes, using your back muscles instead of your leg and thigh muscles to lift large, heavy objects can lead to a herniated disk, as can twisting and turning while lifting. Most cervical problems are due to degenerative changes that occur in the discs and joints of the neck. Bone spurs that stick into the spinal canal take up space, making the spinal canal smaller. They can press against the spinal cord or nerve roots.

Pressure on the spinal cord from spinal stenosis can cause symptoms of myelopathy. Myelopathy may impair normal walking, hand and finger use, and bowel and bladder function. Doctors take these symptoms very seriously because severe myelopathy that is not treated may lead to permanent nerve or spinal cord damage. Pressure on nearby nerve roots can cause radiculopathy and may produce pain, weakness, or sensory changes in the area supplied by nerves that go from the cervical spine to the shoulder, arm, or hand.


Epidural Steroid Injection (Nerve Block)
If other treatments do not relieve your neck pain, you may be given an epiduralsteroid injection (ESI), also called a nerve block. An ESI places a small amount of cortisone into the bony spinal canal. Cortisone is a strong anti-inflammatory medicine that may control the inflammation surrounding the nerves and may ease the pain caused by irritated nerve roots. This injection is often used when other conservative measures do not work, or in an effort to postpone surgery. This treatment is not always successful but may provide short-term help. Physical Therapy
If your condition is causing only mild symptoms and does not appear to be getting worse, your doctor may have you work with a physical therapist. A well-rounded rehabilitation program assists in calming pain and inflammation, improving your mobility and strength, and helping you do your daily activities with greater ease and ability.

In mild cases, nonsurgical treatments offer ways to control symptoms and enable you to improve without surgery. You may be issued a neck brace to hold your neck steady, reducing extra strain. You will need to limit your activities, especially heavy and repeated movements of the arms and neck. Therapists use electrical stimulation to ease muscle spasm and pain. Treatments may also include cervical traction to gently stretch the joints and muscles in the neck. Therapy sessions may be scheduled two to three times each week for up to six weeks.

Surgical Treatment

Cervical myelopathy can be a serious problem. The pressure on the spinal cord usually will not go away without surgery, and the symptoms may continue to get worse. If you do not improve with non-operative care, your doctor may suggest surgery.
There are several surgical procedures used to treat cervical spinal stenosis that is causing cervical myelopathy; all have the same goal-to relieve the pressure on the spinal cord by making the spinal canal larger.

Your surgeon may suggest an operation called a laminectomy, in which the backside of the vertebrae is opened to allow more room for the spinal cord. Discectomy, the removal of one or more discs, may be suggested if stenosis is coming from problems of disc herniation. This surgery may be done from the front or back of the spine. Or your surgeon may suggest an operation that is done from the front of the neck-a corpectomy and strut graft. This operation involves removing the discs and vertebral bodies in the area where problems are occurring. Bone spurs that are pushing into the spinal cord are also removed. The vertebrae are then replaced with a solid piece of bone graft (called a strut graft). The strut graft heals over time to create a solid fusion of the spine where the vertebral bodies have been removed.
